Dear Parent or Guardian of Sixth Grade Students:

New York State law now requires that all children entering sixth grade who are 11 years old receive a Pertussis booster in the form of a Tdap immunization.

If your child is 11 years old or will be by September, this vaccine must be given BEFORE entry into sixth grade. A certificate from your doctor, verifying the date of the immunization or the date of the last DPT, if given within 2 years, MUST BE PRESENTED ON THE FIRST DAY OF CLASS OR YOUR CHILD WILL BE EXCLUDED FROM SCHOOL. Please note that if your child has not yet reached their 11^th birthday by September then they may attend school with the understanding that they must receive this immunization within 14 days of their 11^th Birthday. Alternatively, students can receive the Pertussis vaccine Boostrix which can be given to children who are only 10 years old (this will satisfy the requirement).

Please check with your physician and make the necessary arrangements to avoid any exclusion in September and present the necessary certification to the nurse’s office on the first day of school.
