Registration Form The registration form must be completed and signed by the parent(s). You must provide your child’s original birth certificate (not copy) or other proof of his/her birth date (e.g., passport). If the birth certificate is not in English, a notarized translation must be submitted. You must provide proof of your child’s immunizations (which must be signed by a physician). For Homeowners: You must provide a copy of one document from both categories 1 and 2 below. - Closing papers indicating ownership, deed, recent mortgage statement or property tax bill
AND - Utility bill such as gas, electric, or water bill (phone bills are not accepted). Or a voter registration card.
| For Renters/Non-Owners: If you are renting or leasing, submit the following: - A copy of the written lease or rental agreement (if applicable)
- A completed, notarized Renter’s /Non-Owners Affidavit
- A completed, notarized Owner’s Affidavit
- A copy of the deed, recent mortgage statement or property tax bill. Owners may mail or fax this to the appropriate school. (See reverse side for mailing or faxing information).
| For Divorced or Separated Parents: 1. Provide a copy of the current court order or divorce papers or separation agreement pertaining to custody of the child you seek to register. 2. If there is no current custody order, the custodial parent must provide a completed, notarized “Custodian’s Affidavit.” For Legal Guardians or Custodians: 1. Provide a copy of the court order awarding custody. 2. If there is no court order, you must submit a completed, notarized “Parent’s Affidavit” and “Custodian’s Affidavit.”
For Foster Parents: Submit a copy of the BSW-241 or DSS2999 form.
If applicable, please provide copies of any current orders of protection concerning your child(ren).
